Sarah and Ephraim were both originally buried in the Shelby Family Burying Ground in Lincoln County, Kentucky. Their remains were reinterred in 1879 in McDowell Park, which was created and dedicated in Dr. McDowell's honor by the Kentucky Medical Society on December 13th and 14th, 1879.
Four of their children remain interred in the Shelby Family Burying Ground in Lincoln County.

Portions of the cemetery were relocated to other sections in 1994 due to church expansion. Sarah McDowell is not listed in the relocation records, indicating that her marker was illegible at that time.
