John (João) Baptista was born in Agua de Pau on the Azores' island of Sao Miguel. In the hand-written birth record in Sao Miguel, he was named João de Sousa DaMazio Baptista and christened on December 25, 1873 at Nossa Senhora dos Anjos. Hi death certificate recorded his name as João Souza deMazio known as John Baptista.

According to cemetery records, he is buried under the name John B. Souza in Lot #191/Grave 2/St. Ann (Sylvia plot). The plot was owned by Manuel J. DeSilvia at the time of burial. I could not locate a grave marker for him, and I even dug a little with a stick in the area he is buried when I spotted what appeared to be pieces of stone that may have been part of a grave stone. I finally gave up.

His name is spelled differently on various public records, such as census, state marriage and death records, and city directories. These name spellings have included his first name as João or John, and his last name as Baptista, Baptiste, Souza, and deMazio.

João's widow, Maria is buried as Mary Baptiste in the Coute plot at: Saint Kevin, Lot 141.
