Son of David Lowry Swain and his wife Eleanor Hope White Swain. His father was a former North Carolina Governor and was the long-time President of the University of North Carolina.

He was a graduate of the University of North Carolina (Class of 1858) and of the Medical College of South Carolina in Charleston, S.C. He served as a surgeon in during the Civil War with the 39th Regiment of N.C. Troops.

At the urging of his brother-in-law, General Smith D. Atkins of Freeport, Dr. Swain moved to Shannon, Carroll County, IL in 1868 with his wife, Margaret Louisa Steele Swain, and their daughter, Eleanor Louise Swain. Dr. Swain practiced medicine until his premature and accidental death in 1872.

His widow remarried and is buried in Asheville, NC under the name "Margaret L. Steele." His daughter Eleanor Louise "Lula" Swain was living in Shelbyville TN in the early 1880s. She inherited land in Buncombe County, NC from her grandmother Swain. She is buried in Asheville, NC under the name "Louise Swain Clayton."

Some of the information about his family comes from the case of Clark v. Atkins decided by the N.C. Supreme Court in 1884 and from the book "Undaunted Heart: The True Story of a Southern Belle and a Yankee General" (Suzy Barile 2009).
