Some family records show Ruth Ballard's name as Alma Rosetta (McCoy) Ballard. However, LDS records correlate to those on her Find A Grave memorial page. The origin of the "Johnston" part of her name is unknown to her family. Her brothers and half sisters and their families knew her as "Alma." Her last husband's family knew her as "Ruth."

Ruth Ballard's grandmother, and her father, Joseph McCoy's mother, Ada Arabella Harris (#105839091), was a full blood Chickasaw Indian. Her mother was of English-Irish ancestry. Ruth Ballard was not registered in the Indian Territory rolls, since she was born after the statehood of Oklahoma on November 16, 1907.

Alma Ballard was first married (June 24, 1938) to Lee Prescott Mathis (# 49622415), and they had no children. Her second husband was "JC" Ballard (married October 27, 1955), and they also had no children. She had a daughter at a young age prior to her first marriage named Rosemarie (Tullos?) Davis, who who was raised by her cousin, Dorothy (Tullos) Jones and her husband. She died in Florida circa 2002.

Alma Ballard had two brothers, Edwin Gerald McCoy, Sr. (#75676313); and Henry Curtis McCoy, November 25, 1907 - February 16, 1978, location of burial unknown (#181208847).

Alma Ballard had four half sisters: Johnny Pauline Brown (#120862585), Emily Pearl (Brown) Lievsay (#76849997), Vera Beatrice (Brown) Albert (#48874336), and Ruby Jean (Brown) Albert (#81194500). An additional half sister was stillborn (#106283656).
