Dr. Berry was a doctor of family practice at Physicians Primary Care in Jeffersonville, Indiana and was the former band director at Heath High School in Kentucky. He was a graduate of the University of Louisville where he held a doctorate degree in medicine. He was an avid bird watcher and a member of the Audobon Society and was of the Presbyterian faith. He will be remembered by all who knew him as a great husband, father, grandfather and friend.
Survivors include his wife of 29 years, Shirley Berry; two sons Joe Rhea of Reidland and Dan Rhea of Hong Kong; two daughters, Marty Wallace of Lacenter and Lee Ann Puckett of Henderson; twelve grandchildren and eight great grandchildren.
In addition to his parents, he was preceded in death by one great grandson, Jessie Mac Puckett.
Funeral services will be held at 11:00 a.m. Saturday, December 3, 2011 at Hughes Funeral Home of Paducah with Rev. Stan Durrett officiating. Burial will follow at Maplewood Cemetery in Mayfield.
Visitation will be from 5:00 p.m. until 8:00 p.m. Friday at the funeral home. A memorial service for Dr. David F. Berry Jr., M.D., will be at 11 a.m. Saturday, Dec. 10, in the lobby of Kentuckiana Medical Center, 4601 Medical Plaza Way, Clarksville, just off of Veterans Parkway at Exit 5 on Interstate 65 North. He died Nov. 29, 2011, in Charlestown. Since he was from Paducah, his funeral was held there on Saturday, Dec. 3, with interment at Maplewood Cemetery in Mayfield, Ky.
