First Lieutenant Janovsky was the pilot of B-25D Mitchell #41-29816. The fifth in a group of ten
B-25s taking off from the Columbia Army Air Base, South Carolina, for a training circuit, the twin engine bomber experienced a failure of the port engine. The pilot banked to the left (into the dead motor) in an attempt to immediately return. But the left wing dipped, and the plane collided with trees and crashed. Five aboard were killed. Two crew members were thrown from the wreck and survived, but with serious injuries.

2nd Lt. Clair E Lewis Jr, Co-pilot
S/Sgt. Howard A Ferguson, Flight Engineer
S/Sgt. Edward A Witt, Radio Operator
S/Sgt. Henry Burt Evans, Gunner
