Staff Sergeant Horak was the Tail Gunner on Lt. James Roden's B-24 crew. They were assigned to the 788th Bomber Squadron, 467th Bomber Group, (Heavy), U.S. Army Air Forces, stationed at Rackheath Air Field, England. He was killed when their aircraft was shot down while returning from a mission over Hamm, Germany. German fighters had followed the group of 28 aircraft back to England and, as they made their approach for a night landing, shot down two aircraft killing 17 crew members and one airman on the ground.

Sgt. Horak was awarded the Purple Heart Medal; the European, African, Middle Eastern Campaign Medal; and the World War II Victory Medal.

He entered the service from Minnesota.
