The 133rd Ohio mustered out of service on August 20, 1864 at Camp Chase, Columbus Ohio after 100 days service. Major Clark is listed on the official roster as having mustered out with the unit at that time. His death, as noted on the grave marker occured 11 days later at Fortress Monroe where he was hospitalized in Virginia.
