Most researchers spell his last name with an S, but obviously at the time of his death, it was thought to be spelled with a C.
DOB and DOD are from other sources, the DOB on the stone appears to be June 13th 1788 and the DOD appears to be December 2. 18.. (The 18 is under the "m" of the part of December you can see - 'cem' on left piece and 'ber 2' on right piece.
