David was born in 1835 to Roswell and Polly Aylsworth in New York State. On January 22, 1854 he married Catherine Nevel, daughter of Clemson and Sarah Nevel. Of this union there were 4 children born - George, Sarah, Edward and Lucy.

David was a lawyer in Richland Center, WI. When the war broke out David enlisted in Co. D 11th Wisconsin Infantry on September 12, 1861. He died on August 17, 1864 in Richland Center, WI, of disease contracted during his service years. For his service record, his last name is listed as Ailsworth or Aylsworth.

His headstone is behind the Civil Ward cannon mounted monument.
