Declaration
In order to obtain the benefit of the Act of Congress of the 7th July, 1838, entitled "An Act granting half pay and pensions to certain widows."
State of Maine
Franklin County.
On this second day of August A.D., 1838, personally appeared before the Hon. Nathan Weston, Chief Justice of the Supreme Judicial Court of the State of Maine, Susanna Church, a resident of Farmington in the County of Franklin and State of Maine, aged eighty two years, who, being first duly sworn according to law, doth on her oath make the following declaration, in order to obtain the benefit of the provision made by the Act of Congress, passed July 7th, 1838, entitled "An Act granting half pay and pensions to certain widows;" that she is the widow of John Church late a revolutionary pensioner of the United States under law of congress of June 7, 1832. She refers to the evidence furnished the war department by her late husband of his services in the army of the Revolution in order to establish his claim to pension under said law.
She further declares, that she was married to John Church on the eighteenth day of May, in the year seventeen hundred and seventy eight; that her husband, the aforesaid John Church, died on the twelfth day of March 1838; that she was not married to him prior to his leaving the service, but the marriage took place previous to the first of January, seventeen hundred and ninety four; viz. At the time above stated; that she has remained a widow ever since that period, as will more fully appear by reference to the proof hereto annexed.

Sworn to and subscribed, on the day and year above written, before me (the applicant being unable from infirmity to appear in open court).
/s/ Nathan Weston, C.J., S.J.C.
State of Maine
On the second day of August, 1838, Susanna Church, the applicant within named, was examined and sworn, as within stated. She exhibited to me their family record, of the authenticity of which I have no doubt, from which it appeared, and I am satisfied of the fact, that she was married to John Church, late a revolutionary pensioner of the United States, on the eighteenth day of May, 1778. And I am further satisfied, that the said John Church deceased on the twelfth day of March 1838, and that the said applicant has ever since remained his widow.
