Hazel was born August 20, 1925 to James and Rose Guymon in Fountain Green, Utah. Married Avon R. Park on July 12, 1947. She passed away on June 25, 2011 in Salt Lake City. She resided at 973 West 800 South in the Glendale neighborhood of Salt Lake City, Utah at the time of her death.

Hazel is preceded in death by her husband of sixty years. Faithfully served as one of Jehovah's Witnesses for fifty-six years.

She is survived by: her five children, Ellen Weaver, Shana (George) Hassler, Gary (Bonnie) Park, Linda (Mark) Hyland, and Scot (Shelly) Park; thirteen grandchildren; seventeen great-grandchildren; and one great-great-grandchild.

The family expresses great thanks for the care mom received from the caring individuals at CareSource and Beehive Homes.

Graveside Services will be held on Friday, July 8, 2011 at 11:00 a.m. at Redwood Memorial Estates Cemetery, 6500 South Redwood Road (1700 West), Taylorsville.
