Death certificate: Mary Jemima Lewis, b. Aug. 25, 1877, Utah, parents unknown, widowed, d. Los Angeles County General Hospital, usual res. 6559 North Figueroa Street, res. in Los Angeles Co. and CA 42 years, informant Hector T. Cavaness, myocardial infarction, bur. Forest Lawn Cemetery. Per the 1900 census, she was b. Aug 1878.

"Mamie Fairchild" m. "Silas E. Cavaness" on Feb. 15, 1894, Juab Co., UT (Utah, County Marriages, 1887-1937). In the 1920 census (Maricopa Co., AZ), Silas Cavaness is living with his sister Rebecca Peeples and is reported as divorced. In the 1920 census (Graham Co., AZ), Mary (as "Mamie") is found married to William A. Lewis and with son Hector Truman. Later that year they moved to Los Angeles, where William Lewis died and Mary continued to live with son Hector Truman Cavaness.
