According to his death certificate, he is the son of Daniel Bailey and Lucy Lamox (both born in North Carolina), and married to Lillie Bailey. He was a minister. The informant was Mrs. D. A. Bailey. His name is spelled Adison on the certificate.

According to the US, World War I Draft Registration Cards, 1917-1918, he was born in Eagle Springs, North Carolina. He was a student of Theology. He was 29 years old and single at the time. He signed the card Addison (D. A.) Bailey.

He married Lillie Hazel Staley on 22 September 1923, in Moore County, North Carolina, and they are the parents of at least the following:

1) Daniel Edwin Bailey, Sr. (1924-2006)
2) Maynard Sheldon Bailey (1926-1945)
3) Nina Jean Bailey (1931-194)

According to the US, Headstone Applications for Military Veterans, 1925-1963, he enlisted as a private on 4 October 1917, in North Carolina. He served in Company I 120th Infantry, 30th Division. His headstone was shipped to Fairview Memorial and the application was signed by Mrs. D. A. Bailey.

Note 1: The death certificate says the cemetery is Eagle Springs, however the original information for his memorial came from the Fairview Memorial Park burial records (his headstone application agrees with the burial record).

Note 2: Doctor is his first name, rather than a title. Per the 1900 and 1910 census records when he was 12 and 22 years old, his first name is Doctor A. Bailey. Per the 1930 census record and his draft registration, his name is Addison Bailey. Per the 1940 census record, his name is back to Doctor A. Bailey, which matches his headstone.
