Note 1: The death certificate says the cemetery is Eagle Springs, however the original information for his memorial came from the Fairview Memorial Park burial records (his headstone application agrees with the burial record).
Note 2: Doctor is his first name, rather than a title. Per the 1900 and 1910 census records when he was 12 and 22 years old, his first name is Doctor A. Bailey. Per the 1930 census record and his draft registration, his name is Addison Bailey. Per the 1940 census record, his name is back to Doctor A. Bailey, which matches his headstone.
