[?] N_______ [?], M.D., who treated her from 03 OCT to 24 NOV 1918; interred 26 MAY 1918 at Riverdale Cemetery by undertakers Herring & McGehee of 1400 Broad street, Columbus - per death certificate #1200 (informant Joel EVERIDGE of 3613 Fifth Avenue, Columbus).
