He died at 11:55 a.m., Saturday, August 4, 2011 at his White Oak, West Virginia residence following a short illness. He is the son of the late Melvin Ballard and Eva Reed Ballard.
He was a disabled coal miner.
Survivors include his wife, Kathy Hall Ballard; two daughters, Teresa Honaker and Vicki Kincaid (husband Charles); a brother, Roger Ballard (wife June); and six grandchildren, David, Chrystal Lynn. Jay, Tonya, Tiffany, and Chrystal.
Service at 11:00 a.m. Monday, August 6, 2001 at Rose and Quesenberry Shady Spring Chapel with the Rev. Allen Terry officiating. Burial at Blue Ridge Memorial Gardens at Prosperity, West Virginia.
