Sarah Elizabeth Alexander, born in 1866, was the daughter of Elizabeth Ann (née Zittle) and Jacob Michael Alexander. She was named after her maternal grandmother and mother. Her father appears to have died before 1880.

Sarah Alexander and Frank Thomas were married on 27 November 1895 in Benevola, Washington County, Maryland. Their son Harry was born the following year. In 1900, as "Elaxander, Sallie," she was enumerated in the Boonsboro household of her aunt Mary E. Zittle, with son Harry Thomas. Although Sarah is marked as being married (for "3" years), her husband is not found in the 1900 U.S. Census.

Neither Sarah nor her husband Frank are found in the 1910 U.S. Census. Son Harry was then living with his great-aunt Mary E. Zittle, and resided with her when she died in 1916.

Sarah Elizabeth Thomas died in 1915 at the age of 49. One death notice mentions her work as a nurse at Bellevue. This was the Washington County Alms House in Hagerstown, where her mother was a warden for 10 years.
