Jacob was the son of Jacob Cook and Phebe Lindsly. He was probably born in Mendham, New Jersey as was assumed in the case of all his siblings. His will was signed March 27, 1782, naming his father Jacob Cook, as executor. He died unmarried. Jacob Cook, Sr was sworn in as executor on November 26, 1782. Will Book I, Courthouse, Washington, Pennsylvania. His will provided money for the education of his sibling's children, thus providing the names, or inferred names of some siblings or their spouses if the siblings were deceased-siblings that were not named in his own father; Jacob Cook Sr's will.

One of these inferred names was Susannah Cook Frazee who married Benjamin Frazee. Benjamin is named in Jacob Cook, Jr's will where Jacob provided for the education of the eldest son of each of Jacob's siblings. She was not named in her father's will. She died in 1790.
