There was no official death certificate form completed and filed. The following is the "Standard Certificate of Death" #19902, that was accepted by Collin County as proof of death:
"Plano, Texas, October 8, 1914
County Clerk, McKinney, Texas
"Dear Sir:
Not having a death certificate - and you may send me a few - I report the death of Mrs. Evelina Behn (colored), age 24+, married--sick about 2 weeks, died October 4, 2 a.m. of typhoid fever and malaria--residence near Plano, Collin County, Texas."
