She was born in Maysville, Kentucky on September 17, 1911 the daughter of the late Grover and Mary Haughaboo Griffith. She was a retired Ruth Hunt Candy employee and a member of the First United Methodist Church. She was the widow of Leslie Forman.
She was also preceded by a daughter, Phyliss Forman Garrison; two brothers, Jack and Charles Forman; and two sisters, Evelyn Smith and Gladys Hillis.
Funeral services were 1 PM, Tuesday, January 3, 2011 at the Taul Funeral Home Chapel in Mt. Sterling
