100, of West High Street, Mt. Sterling, Kentucky passed away Friday, December 30, 2011 at the Fountain Circle Rehabilitation Center in Winchester. She was born in Maysville, Kentucky on September 17, 1911 the daughter of the late Grover and Mary Haughaboo Griffith. She was a retired Ruth Hunt Candy employee and a member of the First United Methodist Church. She was the widow of Leslie Forman.
She was also preceded by a daughter, Phyliss Forman Garrison; two brothers, Jack and Charles Forman; and two sisters, Evelyn Smith and Gladys Hillis.
Funeral services were 1 PM, Tuesday, January 3, 2011 at the Taul Funeral Home Chapel in Mt. Sterling
